Here is a form to complete regarding consent to Leucan’s family services and the use and transmission of personal information. When a person is 14 years of age or older, he or she can consent to their own care (with some exceptions).
Here is an overview of the services offered to Leucan families:
- Emotional support and guidance
- Massage therapy
- Socio-recreational activities
- School awareness program and support
Use of personal information
Personal information is any information about an individual from which that person can be identified. Personal information on file will be accessible to employees who need it for work purposes, and may be disclosed to the following third parties:
- Private partners/professionals (e.g. psychologist, psychotherapist, massage therapist)
- Schools
- Partners in the health and social services network (e.g. hospitals, CLSCs, rehabilitation centres, community organizations)
- Foundations
File information can be communicated to the above-mentioned third parties for the following purposes:
- Any relevant documentation about a member (e.g. evaluation reports)
- Any relevant information about a member’s condition and activities (e.g. medical, family, school)
- Any other relevant and necessary reason that facilitates and contributes to the accompaniment and support offered to members
The information contained in the file will be hosted on Leucan’s secure servers.
